To register a new patient you will need to live within our practice boundary.
Please bring two forms of identification with you to the practice along with the below forms:
- One photographic, ideally your passport or ID driving license
- One recent proof of address e.g. utility bill dated within the last three months or letter from your employer confirming that you have taken up residence in the area for at least six months.
We offer a basic health check to all new patients within 6 months of registration to cover height, weight and blood pressure. Please ask for an appointment with the healthcare assistant.
Medical treatment is available from the date of registration. Please contact reception for further information.
You can register temporarily with a GP surgery while away from home for work, study or on holiday. You’ll remain registered with your permanent GP surgery.
Temporary registration is available for patients temporarily staying in the area (longer than 24 hours but less than 3 months)
